Examples of changes in condition that would indicated a screen include, but are not limited to, the following:
- Falls or changes in balance
- Requiring more assistance/calling for help more frequently
- Using new/different assistive device
- Increased use of wheelchair
- Wheelchair use/instructions or fitting
- Decreased bed transfer ability/overall functional mobility
- Weight/appetite changes
- Decreased safety awareness
- Medication change
- Increased assistance with dressing, showering, or toileting
- Change in participation (meals, groups, exercise, etc)
- Equipment questions or needs
- Residents who are hospitalized or in a skilled nursing facility
- Change in cognition
- Swallowing/Choking concerns
